Disney monorail crash due to human error

The infamous "sources familiar with the situation" have let slip that the monorail crash at Walt Disney World Resort earlier this month was caused by human error.
An employee who was stationed in the monorail's maintenance bay on July 5 was radioed with a request to switch the tracks that would have allowed the train to enter the bay for the night. The employee radioed back that the switch was complete and the train was allowed to continue. The switch, however, had not been completed. The train was on the wrong track when it collided with a stationary train one checkpoint ahead of it. The employee told investigators that he thought the track switch had been completed before he radioed the go-ahead.
Three employees have been fired from the resort in the aftermath of the collision. It is not yet clear if this employee was one of them. [Orlando Sentinel]
